Please note: While a membership is required to access our Primary Care services, Chiropractic and Acupuncture therapies are available through direct access and do not require a membership.
Please note that a Family Membership consists of either 2 immediate family members or 2 adults + 1 child as long as the child is under the age of 18 or is currently covered by parent’s health insurance up to the age of 26.

Dr. Shay Shani and Team do an amazing job. Several of my family members go see him and they love it. I personally think this office makes miracles. I’ve seen it myself. What Makes Our Membership Unique
We understand that moving to a membership-based care system may bring questions. To help, we’ve created this FAQ with clear answers about enrollment, services, privacy, and the financial benefits. Our goal is to provide transparency so you can make an informed decision about your care.
The trend in large medical groups is to put the business side of medicine first. This results in rushed office visits and less time with your Doctor. At Summit, we insist on preserving the doctor/patient relationship. We have a patient-centered experience that is hard to find – customer service, easy scheduling, added services and extra support are the standard. The membership allows us to maintain this standard
Our membership rate was established 6 years ago. It is a fraction of the cost of
any MD-VIP or Concierge Practice who charge upwards of $2000 per patient per year.
Any unused portion of the fee will be prorated and refunded if you leave the
practice for any reason.
Yes, you can request a Payment Plan and divide the membership fee in either
3 or 6 consecutive monthly payments. We do require a credit card on file which will be automatically charged each month until the membership fee is paid in full. If you are interested in this option, please email us.
