COVID-19 VACCINE UPDATE

Dear Summit Health Patient,

 

Beginning March 15th, the Vaccine Roll-out will open to individuals ages 16-64 who are deemed high risk. Per the California Public Health Department, the specific severe health conditions are as follows:

  • Cancer, current with debilitated or immunocompromised state

  • Chronic kidney disease, stage 4 or above

  • Chronic pulmonary disease, oxygen dependent

  • Down syndrome

  • Immunocompromised state (weakened immune system) from solid organ transplant

  • Pregnancy

  • Sickle cell disease

  • Heart conditions, such as heart failure, coronary artery disease, or cardiomyopathies (excludes hypertension)

  • Severe obesity (Body Mass Index ≥ 40 kg/m2)

  • Type 2 diabetes mellitus with hemoglobin A1c level greater than 7.5%

  • Due to a developmental or other severe high-risk disability one or more of the following applies:

    • The individual is likely to develop severe life-threatening illness or death from COVID-19 infection

    • Acquiring COVID-19 will limit the individual's ability to receive ongoing care or services vital to their well-being and survival

    • Providing adequate and timely COVID care will be particularly challenging as a result of the individual's disability

 

 

The County will require documentation from your Primary Care Provider upon arrival for your 1st vaccine. With this expansion of vaccinations, we are expecting a large number of calls from our patients. To accommodate everyone in a timely manner, we ask that you follow these instructions:

 

  • Please ONLY call our office to schedule an appointment for your Doctor's letter once you have your 1st vaccine scheduled. We will ask the date, time & location of your vaccine when you call us.

  •  Unfortunately, scheduling your vaccination does not mean all patients will be given a letter. We must go by the criteria set by the CA Dept of Public health as listed above. If you don’t meet the criteria, please reserve the appointment for others.

  • A 15 minute Video or Phone Appointment with Dr Wilkes or Carrie Sadler will be required to obtain your letter. Our staff will email you the signed documentation within 24 hours after your appointment.

  • We ask that you call us no more than 5 days prior to your 1st vaccine. Do NOT wait until the last minute to call us. We will not be able to accommodate your request if you contact us within 24 hours of your vaccine appointment.

  • This applies to our established Primary Care Patients. If you only see Chiropractic &/or Physical Therapist, you will need to contact your Primary Care Provider for the required documentation.
